Please contact Human Resources to obtain the proper forms to request a leave of absence.
All leaves are subject to approval based on the Minnesota Department of Labor (MDOL) standards and the Family Medical Leave Act (FMLA) guidelines established by the federal government and state in which the leave is being requested. Leaves are also subject to Supervisory and Board approval.
Am I eligible for FMLA approved leave of absence?
The Family and Medical Leave Act (FMLA) and the MDOL standards are in effect for all State employees with 12 months of service and 1250 hours in the previous 12 months.
This Act entitles you to take up to 12 weeks of unpaid, job-protected leave each year for specified family and medical reason such as: personal serious health condition, or to care for a seriously ill child, parent, or spouse, or the birth of a child.
The important difference between FMLA leave and other kinds of unpaid leave is that the employer is required to continue to pay the employer’s share of your health, dental, and vision premiums for the duration of the leave.
See FMLA employee guide for more details regarding the FMLA leave act.
